TEAM CGM: Team Support to Improve Glycemic Control Using CGM in Diverse Populations

This study evaluates if continuous glucose monitoring (CGM) can improve glycemic control in the primary care setting, aimed at individuals with poorly controlled type 2 diabetes (T2DM). We will study 318 T2DM patients receiving pharmacist- and community health worker (CHW)-supported CGM delivered through community health centers (CHCs) in Massachusetts (Baystate and UMass Memorial).
