Improving medication reconciliation in Internal Medicine

Description
Medication reconciliation is a critical step to providing high quality care in the inpatient setting. Despite this, medication reconciliation on admission is often delayed or missed altogether. This leads to errors in treatment, poor communication with patients, and potentially heightened risks for major complications. A group of internal medicine residents is working to measure and improve time to reconciliation in patients admitted by residents using multiple interventions. Students will collect and analyze time- to-completion data reports and will have opportunities to help build educational materials, extend the intervention to medical students rotating on internal medicine services, and potentially contribute to or join presentation or publication efforts.
