Description
We are developing a sub-clinic at APW (in collaboration with the Road to Care team) to track testing and management of Hepatitis C and STDs amongst patients that are screened during their visit. The project includes clinical engagement every week and patient outreach. Patients are recorded and tracked on REDCap and EPIC with appropriate follow-up for education purposes. Test results and treatment for Hepatitis C is also tracked.
Goals
1. Determine the efficacy of intervention and patient education for Hepatitis C screening and treatment in high-risk populations at APW
